This online course is an application-based activity.

Overview
This online, self-study course focuses on planning, implementing, and maintaining a successful diabetes self-management education and support (DSMES) program. It applies to all DSM delivery models, including physician-based, hospital out-patient-based, or pharmacy-based programs, and independent practitioner programs.

This course's five modules review the six main components of a professional DSMES program business plan:

• Module 1: Preliminary Key Activities
• Module 2: Operational Planning
• Module 3: Marketing Plan
• Module 4: Financial Plan
• Module 5: Quality Management and Clinical Plan

Purpose
The purpose of this activity is to enable the learners to grow their business knowledge in order to start and operate a successful DSMES program.

Continuing Education Credit
This educational program will provide 6 contact hours of continuing education credit. It is an application-based activity.

Learning Objectives
1. Prepare a Needs Assessment and SWOT Analysis and then apply the results to decide whether to open a new DSMES program and/or continue to operate an existing program
2. Formulate S.M.A.R.T goals (Specific, Measurable, Attainable, Relevant, Time-based goals) for a DSMES program
3. Describe the components of a successful project management plan designed to guide the implementation of a new DSMES program or make improvements to an existing program
4. Identify the requirements of an operational plan for a successful DSMES program
5. Design a marketing plan for a DSMES program
6. Review strategies for increasing patient referrals and attendance
7. Analyze revenue sources, rates, and financial reports and projections as part of constructing a financial plan for a DSMES program
8. Propose measures to track and evaluate participant and program outcomes

Learning Materials
All course materials will be provided online. No textbook purchase is required. Downloadable resources are included, to assist learners in their own DSMES programs.

Intended Audience

The AADE Webinars are knowledge-based activities designed for individual or groups of diabetes educators, including RNs, RDs, Pharmacists, Nurse Practitioners, Clinical Nurse Specialists, Physician Assistants, MDs, and other health care providers interested in staying up to date on current practices of care for their patients with diabetes and other related conditions.

Registered Nurses

The American Association of Diabetes Educators is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.

This educational program will provide 6 contact hours of continuing education credit.

AADE is accredited as a provider of continuing nursing education by the California Board of Registered Nursing (CEP # 10977).

Registered Dietitians

The American Association of Diabetes Educators, (AM001) is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR). Registered dietitians (RDs) and dietetic technicians, registered (DTRs) will receive 6 continuing professional education units (CPEUs) for completion of this program. Continuing Professional Education Provider Accreditation does not constitute endorsement by CDR of a provider, program, or materials.

Registered Pharmacists

The American Association of Diabetes Educators is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.  This program provides 6.0 contact hours (0.60 CEU’s) of continuing education credit.
